How to Schedule Your Evaluation

Scheduling your presurgical psychological evaluation at Creekside Psychology is straightforward. Whether you have been referred by your surgeon, primary care provider,  or are reaching out on your own, we are here to guide you through the process.

Step 1: Schedule

Call us at (206) 931-3793 or email coy@creeksidepsychology.com to request an appointment. Our team will answer your questions and help you find a convenient time.

Step 2: Complete Intake Paperwork

Before your appointment, you will receive intake forms to complete. Filling these out in advance helps ensure your evaluation time is used effectively.

Step 3: Verify Insurance or Form of Payment

Step 4:  Reminders

You will receive appointment reminders via text..  You can always reply STOP to opt out or reply HELP for assistance.  Message and data rates may apply.

Step 5: Log-in for Your Evaluation

You will receive a link to log-in for your telehealth evaluation.
